This document does two jobs that are usually sold separately. First it appoints a healthcare agent: the one person given authority to make medical decisions on your behalf while you are unable to make them yourself. Second it records the advance directive, your own instructions about the care you would and would not want, so your agent is carrying out your decisions rather than making them up under pressure.
That second part is the one worth taking seriously. An agent with no instructions is being asked, in a corridor, at two in the morning, to guess what you would have chosen and then live with having guessed. Families that disagree in a hospital are almost never disagreeing about who to trust. They are disagreeing about what you would have wanted, which is a question only you can answer and only in advance.
It applies only while you cannot decide for yourself. As long as you can express a decision, yours is the one that counts, and your agent has no authority at all. This is not a document that hands over your care. It is a document that describes your care when you cannot describe it.
